Fee and AHCIP context
Alberta lists medical forms and notes among services not covered by AHCIP. A physician decides whether any assessment is medically necessary and insured; a third-party document is separate.

Payment after assessment. The public request form does not collect payment details. The clinic handles payment through its approved process after the physician assessment.
Ask before assuming. A request does not guarantee a note, fee outcome, employer or school acceptance, or any other result.
Timing information
For non-emergency documentation requests received by 8:00pm, AlbertaMD aims to complete an eligible note within two hours when a physician decides it is clinically appropriate and the workflow can support it.
This is not guaranteed. A physician may need more assessment, an in-person visit, urgent care, another care setting, or more time.
Requests received after 8:00pm do not use the two-hour target. The clinic operates daily from 8:00am to 10:00pm Mountain Time.
A sick note is for official documentation only. It is not medical treatment. If you need treatment, see an in-person doctor or urgent care; for a medical emergency, call 911 or go to the nearest emergency department.
Prepare for the right conversation
The generic public form remains for callback, Alberta location, and eligibility only. It is not a treatment, symptom, payment, or document-upload form.

A sick-note request does not replace an assessment for treatment. If you need treatment, use an appropriate in-person doctor or urgent-care setting.
An employer, school, insurer, or other organization decides its own requirements and whether it accepts a document.
