You confirm that the information is submitted for callback-request review and that the clinic may contact you using the details you provide. The request records basic callback, current-location, coverage-status, and callback-preference details so the clinic can review the request and contact you. Health details are discussed during the clinic conversation, not entered on the public form.
A callback request sends your practical details for clinic review. The clinic may later explain the appropriate next step; payment, coverage, prescriptions, documents, and clinical outcomes each require the appropriate workflow and review.
Do not use this form for an emergency or severe, rapidly worsening symptoms. Call 911 or seek immediate in-person care. The clinic may determine that another care setting is safer or more appropriate.
If the clinic offers a virtual visit after review, it should provide any additional clinical, identity, privacy, billing, and virtual-care consent information required for that visit.
