Callback-request privacy
When you use the live callback form, AlbertaMD collects the information you submit so the clinic can review the request and contact you. This request starts clinic review, and no payment is collected.
The live callback form collects your first and last name, callback phone number, optional email address, current Alberta location, and health-card coverage status. It does not collect date of birth, symptoms, medications, allergies, health history, health-card numbers, card images, Alberta Wallet details, documents, or payment details.
The information is placed in AlbertaMD’s restricted callback-request queue so an authorized clinic administrator can review basic callback and eligibility details and contact you about the request. It is used only for clinic review and contact about the next step; payment is not collected and clinical outcomes depend on review.
Callback-request information is displayed only in the authenticated administrator workspace. Health details are discussed during the clinic callback, not collected on the public form; urgent concerns should be directed to 911 or immediate in-person care.
This operational privacy notice describes the currently live callback workflow. A clinic privacy lead should still complete and publish a full privacy policy covering retention, access and correction requests, service providers, records management, and the care-delivery workflow.
