Sick Note Calgary – Online Doctor’s Note for Work or School
Calgary patients can book an offered phone appointment through Ava. An Alberta physician assesses whether a sick note is clinically appropriate.
For a possible emergency, call 911 instead of waiting for a phone appointment.
$40, no tax — only if a note is issued following assessment. Payment is handled after physician assessment through the clinic’s approved process. A note is not guaranteed.
Conditional two-hour target. For eligible requests received by 8:00pm, the target applies only when the physician finds the note clinically appropriate and the workflow permits. It is not guaranteed. Requests after 8:00pm do not use this target; booking a future appointment does not start it.
Be physically in Alberta with a valid Alberta Health Care card and active AHCIP coverage. Register, including health-card details, only inside Ava. General clinic hours: daily, 8:00am–10:00pm MT; actual appointments depend on Ava’s offered slots.

How to get a sick note online in Calgary
- 01
Book an offered phone appointment
Choose a time in Ava for today or an upcoming day, if offered. Complete its required registration there and follow Ava’s appointment status and instructions.
- 02
Discuss the documentation question
Tell the physician when the illness occurred, the dates missed and what the recipient requires. The physician considers whether the available information supports an assessment and an accurate note. Further information or in-person care may be needed.
- 03
Receive a note only if appropriate
If a note is issued, the approved process handles payment after assessment. A PDF may be emailed after the required consent and identity verification. Booking, payment or a note does not guarantee recipient acceptance.
Prepare for the assessment
Sick notes for work and school
Check what the recipient needs
Ask your employer or school about its current absence policy, required dates, forms and deadline. Not every absence requires a note, and a document does not guarantee a work-leave decision, extension or deferred exam.
Calgary students should check their own school and course rules. The University of Calgary publishes a course-interruption process; a note cannot be required for an in-course assessment absence under that process.
Read UCalgary’s course-interruption informationWhat a medical note may contain
Depending on the assessment and purpose, a note may describe a medically supported period of absence or an anticipated return date. The clinician determines the appropriate content. A basic sick note does not promise return-to-work clearance, accommodation documentation or a specialized form.
Workplace medical information should be relevant to the purpose. Alberta Human Rights Commission guidance says a diagnosis is generally not required except in limited circumstances.
Read Alberta workplace medical-information guidanceKeep absence dates and recipient requirements ready for the physician conversation. Do not submit symptoms, diagnoses, medication lists, health-card numbers, photos, records, forms, or payment information on this website. Follow Ava’s registration, booking status, and instructions there.

Past illness dates and situations that need more assessment
Can past illness dates be documented?
Tell the clinician when you were ill and which dates were missed. The clinician assesses whether enough information exists to document that period accurately. Past-date documentation is not guaranteed and must not imply that an assessment took place on a day when it did not.
Read the Alberta past-date documentation guideWhat if the physician cannot issue a note?
The physician may be unable to support the requested dates or statement. A virtual assessment may be insufficient, or further information and in-person assessment may be needed. Do not rely on a booking to obtain a specific document.
