01

A phone request is not a document outcome

A phone interaction or online request does not by itself establish what a clinician can accurately document. A clinician needs to decide whether the available information supports an appropriate assessment and whether another setting is needed.

CPSA identifies both phone consultations and sick-note writing among services that may be uninsured. That does not verify an AlbertaMD phone workflow, price, or document result.

02

Check the policy that applies to the absence

An employer, school, insurer, benefit plan, or leave program may have its own current process. A clinic cannot decide whether another organization will accept a particular form or document.

Alberta’s leave rules differ by leave type. Personal and family responsibility leave does not require a medical certificate by legislation, while long-term illness and injury leave has a specific certificate requirement for eligible employees.

03

Keep documentation information limited and relevant

For workplace absences, Alberta Human Rights Commission guidance focuses on information related to the absence, expected duration, work duties, restrictions, or accommodation needs. A diagnosis is generally not required except in limited circumstances.

Keep personal health details and forms for a clinician conversation or other approved clinic workflow. Do not enter private clinical information in the public request form.

04

Use the reviewed request within its limits

For a non-emergency health concern, an AlbertaMD public request starts clinic review. The clinic reviews the request and contacts the person about the next step. It does not confirm an appointment, callback time, virtual visit, sick note, certificate, prescription, coverage, fee, or any other outcome.

For a medical emergency, call 911 or seek immediate emergency assessment instead of waiting for a documentation question or an online request.

FAQ

Common patient questions.

Ready to start? For an eligible non-emergency concern, start your visit form when you are ready; the clinic reviews each request. Call 911 for an emergency or seek prompt professional assessment when a concern cannot wait.

Can a phone request guarantee a sick note?

No. Documentation depends on an appropriate assessment, what can accurately be documented, and the requesting organization’s own requirements.

Can a clinic charge for a phone consultation or sick note?

CPSA says phone consultations and sick-note writing can be uninsured services. This does not establish a fee for any specific clinic; ask the clinic about a current process and any possible fee.

Should a diagnosis appear in work documentation?

Alberta Human Rights Commission guidance says a diagnosis is generally not required except in limited circumstances. Information should be limited to what is reasonably necessary for the work context.
